چکیده

Hypothermia was shown to attenuate ventilator-induced lung injury due to large tidal volumes. It is unclear if the protective effect of hypothermia is maintained under less injurious mechanical ventilation in animals without previous lung injury. Tracheostomized rats were randomly allocated to non-ventilated group (group C) or ventilated groups of normothermia (group N) and mild hypothermia (group H). After two hours of mechanical ventilation with inspiratory fraction of oxygen 1.0, respiratory rate 60 min(-1), tidal volume 10 ml x kg(-1), positive end-expiratory pressure (PEEP) 2 cm H2O or immediately after tracheostomy in non-ventilated animals inspiratory pressures were recorded, rats were sacrificed, pressure-volume (PV) curve of respiratory system constructed, bronchoalveolar lavage (BAL) fluid and aortic blood samples obtained. Group N animals exhibited a higher rise in peak inspiratory pressures in comparison to group H animals. Shift of the PV curve to right, higher total protein and interleukin-6 levels in BAL fluid were observed in normothermia animals in comparison with hypothermia animals and non-ventilated controls. Tumor necrosis factor-alpha was lower in the hypothermia group in comparison with normothermia and non-ventilated groups. Mild hypothermia attenuated changes in respiratory system mechanics and modified cytokine concentration in bronchoalveolar lavage fluid during low lung volume ventilation in animals without previous lung injury.

The effects of volume control inverse ratio ventilation with PEEP on respiratory function and inflammatory cytokines in patients during one-lung ventilation

Background: Hypoxemia is the most common complication during one-lung ventilation (OLV). Inverse ratio ventilation (IRV) may lead to intrinsic positive end-expiratory pressure (PEEP) and improve oxygenation in acute respiratory distress syndrome, so we investigated whether volume-controlled IRV with external PEEP could improve hypoxemia, reduce the risk of acute lung injury during OLV. Methods:...

High tidal volume upregulates intrapulmonary cytokines in an in vivo mouse model of ventilator-induced lung injury.

Mechanical ventilation has been demonstrated to exacerbate lung injury, and a sufficiently high tidal volume can induce injury in otherwise healthy lungs. However, it remains controversial whether injurious ventilation per se, without preceding lung injury, can initiate cytokine-mediated pulmonary inflammation. To address this, we developed an in vivo mouse model of acute lung injury produced b...

Hypothermia Induced by Adenosine 5′-Monophosphate Attenuates Acute Lung Injury Induced by LPS in Rats

We have built a rat's model to investigate whether the hypothermia induced by adenosine 5'-monophosphate (5'-AMP) (AIH) could attenuate acute lung injury induced by LPS in rats. We detected the inflammatory cytokine levels in the plasma and bronchoalveolar lavage fluid samples, and we analyzed the pathological changes in the lungs. We have found that AIH can effectively inhibit acute inflammato...

Genetic and pharmacologic inhibition of Tpl2 kinase is protective in a mouse model of ventilator-induced lung injury

BACKGROUND Mechanical stress induced by injurious ventilation leads to pro-inflammatory cytokine production and lung injury. The extracellular-signal-regulated-kinase, ERK1/2, participates in the signaling pathways activated upon mechanical stress in the lungs to promote the inflammatory response. Tumor progression locus 2 (Tpl2) is a MAP3kinase that activates ERK1/2 upon cytokine or TLR signal...

Recruitment maneuvers in acute lung injury: what do the data tell us?

During the past several decades, we have come to appreciate that mechanical ventilation practices affect outcome in patients with acute lung injury (ALI) and acute respiratory distress syndrome (ARDS). Physiologic evidence suggests that overdistention of alveoli can lead to ventilator-induced lung injury,1,2 and cyclic opening and closing of alveoli is also injurious.3,4 ALI is a heterogeneous ...
